As a dedicated assisted living center, we can offer complete, responsive care to seniors who don’t need full-time skilled nursing. This care includes:

  • Assistance for all ADLs: dressing, hygiene, toileting, mobility, etc.
  • Medication management and in-house lab coordination
  • Incontinence management
  • Nutritious meals, including accommodation of special dietary requirements
  • Daily recreational activities and wellness programs
  • Transportation to and from health appointments
  • Coordination with physical therapists and home health providers
  • Respite stays
  • A safe, secure environment for memory care patients
  • Arrangements for comfort care

New Forest Haven can accept residents directly from the hospital, skilled nursing, home, or another assisted living community. We are able to care for patients with a variety of acuity levels.

As soon as a referral is sent over, a New Forest Haven team member will respond within an hour to assess your patient to determine whether your patient is an appropriate fit for our services.

Upon receiving an inquiry about a patient, we’ll send you the necessary paperwork to sign. If your patient is currently in the hospital we will ask for a patient history, physical, face sheet, list of medications and administration instructions, and a signed admission order.

If your patient is currently in another assisted living or skilled nursing facility, we can obtain documentation from that facility and will only require a new admission order on your end.
